Saint Charles Lwanga America Center
Saint
Charles Lwanga America Center is a resource center promoting opportunities enhancing
housing stability, nutrition security and livelihood productivity. We address is a mental breakdown through the
use of Family Based Therapy Model (FBTM) and other services focused on
evidenced-based, psycho-social-medically driven treatment approaches for
children, adolescents, and adults dealing with mental health conditions both
in-person, via telehealth and through referral networks.
Saint
Charles Lwanga America Center employs psychiatrists, psychologists, psychiatric
nurse practitioners, and licensed therapists to support critical roles which
our space provide better health outcomes through experiential-modulated
activities.
We are seeking
to hire a Part-Time Psychiatric Rehabilitation Program (PRP)
Director/Rehabilitation Specialist who has experience working in the psycho-social
service field and providing case management services. We turned our resource
center into a drop-in/drop-by space for healing.
Responsibilities
We are a
Community-based resource center so we use community-based tools so your
responsibilities are around:
- Manage medical records and
request records from outside agencies.
- Complete and track
individual rehabilitation plans.
- Complete Authorizations via
Optum/Incedo for all adult and child clients in PRP.
- Perform initial and ongoing
staff trainings.
- Review client services to
meet COMAR and billable standards.
- Provide supervision over PRP
Counselors.
- Check for note corrections
and release notes.
- Complete monthly PRP
spreadsheet and billing sheets for all PRP Counselors.
- Schedule and complete new
client intakes.
- Assign new clients to PRP
Counselors.
- Reach out to outside
agencies and the community for new client referrals into the program.
- Mediate client complaints.
- Participate in weekly
multidisciplinary treatment team meetings.
- Engage with on-site/offsite
with clients for emergencies.
- Facilitate PRP All Staff
meetings once a month.
- Respond to and manage PRP
Counselor's concerns regarding case management for any client in PRP, both
adults and children.
- Attend one-on-one meetings
weekly with Chief Executive Officer.
- Maintain accreditation
compliance for PRP and maintain availability for all audits/surveys.
- Additional tasks assigned by
the President in relation to preparing for and partaking in PRP audit when
applicable.
Education:
- Current LCPC or LCSW-C
licensure in the DMV
- Master's Degree in social
work or related field (such as Behavioral Health, Counseling,
- Psychology, Education,
Special Needs/Exceptional Child Education and others...)
