I am introducing Saint Charles Lwanga America Center, a non-profit organization registered with Washington, D.C.US-DC-NP042600187.

We are a behavioral support and modification space.

The Center will be open to beneficiaries during day time referred to us from social services organizations and health facilities. We work hand-in-hand with other social service providers as part of a continuum of care promoting better life outcomes.

While at the Center beneficiaries will be received and assigned activities aligned to needs and in conjunction with what we can offer according to our mission.

Introducing ourselves to you is an action-point we agreed upon following the Faith and Community in Action: HUD’s Continuum of Care Funding Reforms Welcome the Faith Community’s Support in Eradicating Homelessness meeting.

We intend to apply for funding and kindly ask to be provided guidance where possible.

The Founder of Saint Charles Lwanga America Center: Tom Rogers Muyunga-Mukasa

As far as public health and public service are concerned, Tom has led TB elimination drives in the DC/Maryland/Virginia Area and successfully connected 1,000 persons who had a positive TB diagnosis to care. Further to that, Tom has organized over 230 physical meetings at community level where he was able to present health talks to audiences of different age-groups.   This has been possible through networking with other persons and groups. It has not been a one-man show.   Tom's  work has ranged from serving in the USA, Europe, Asia and Africa. The roles are indeed a drum-roll of activities such as implementing, reviewing, monitoring and evaluating activities to mentorship. Most of Tom's work intersects between Clinical,  Public Health and Development Work. Faith, Community in Action, HUD’s Continuum of Care Facilitating Quality Life Outcomes

Working with the Faith and Community entities as enablers of quality life outcomes is a game changer. Quality life outcomes manifest in a wider continuum and can be extended outside of clinical settings.  In a medical setting, health-related quality life outcomes (HRQLO) may be taken to mean medical metrics such as reduced bed admission, reduced bed stays, medical impact on social, mental and physical wellness.  They are vital for gauging healthcare interventions, performance, treatment plans, treatment outcomes and guidance on what to do next as far as referrals and continuum of care is concerned. At the community level these may be prompts for preparedness and readiness to uphold lifestyles that prevent diseases, promote health and prolong life.
